About The Position

The Patient Safety Program Manager is responsible for advancing the culture of safety and ‘just culture’ through strong clinical presence and administration of the safety program on a day-to-day basis. Responsibilities include program oversight, provision of safety assessments and analysis of data, implementation of systems and processes, and coordination of action plans to improve safety. A key component of this position will be conducting patient safety rounds and assessments, ensuring compliance with patient safety-related standards by The Joint Commission (TJC) and other regulatory agencies to reduce risk and improve safety for our patients/residents/clients at assigned HHC facilities.

Requirements

  • Registered Nurse preferred, other clinical healthcare professionals considered.
  • Bachelors of Science Degree is required.
  • Five or more years of health care, quality, safety or risk management experience required, preferably in a hospital setting.
  • Participation in healthcare quality improvement, safety initiatives
  • RN with CT License or relevant healthcare profession license as applicable.

Nice To Haves

  • Master's Degree in Nursing, Health Administration, Management or related discipline strongly prefer
  • CPHQ and/or CPPS Certification is preferred.

Responsibilities

  • Conducts ongoing monitoring of safety practices through assessments, surveys and review of data to identify opportunities for improvement in safe practices and reduction of risks and system-related issues that create barriers to safe practice.
  • Provides patient safety consultation, guidance and expertise to problem solve for system and individual issues for all organization levels. Provides a strong clinical presence and be readily accessible to clinical and non-clinical staff.
  • Leads root cause analysis, failure modes effects analysis, continuous quality improvement and Lean tools to evaluate near-miss and actual events and resolve safety concerns
  • Provides performance improvement support to service lines and actively participate in various committees to support patient safety and performance improvement projects.
  • Designs present and evaluate the effectiveness of educational safety programs used to train all organization levels to help identify and respond to system failures. Educate staff on the safety implications of regulatory issues. HRO trained facilitator.
  • Develop, maintain and update patient safety-related HHC Regional policies and procedures. Collaborate with quality, and Risk department and others as appropriate to ensure a seamless approach to patient safety and risk reduction
  • Actively participate in survey preparation and promote compliance with State, Federal, TJC , and other accrediting agency regulations, standards and survey process. Performs specific performance improvement activities necessary for the clinical and administrative staffs to monitor and evaluate the quality and appropriateness of patient care to meet regulatory requirements (e.g., DPH, TJC, CMS)
  • Provides data abstraction from hospital applications and inputs into internal and external databases to support publicly reported measures and other performance improvement activities.
