About The Position

The Patient Safety Manager will lead, facilitate, and support Root Cause Analysis (RCA) and Just Culture (JC)processes. This may include responding to unanticipated outcomes, meeting with staff involved in the situation, scheduling/planning meetings, mentoring managers, reviewing medical records, documenting meeting minutes, tracking outcomes and process improvements, and trending results. And performs other duties as assigned.

Requirements

  • RN degree from an accredited school of nursing
  • Bachelor’s Degree required, Master’s Degree preferred.
  • Required Licenses[Ohio, United States] Registered NurseLicensed to practice in the State of Ohio
  • Minimum of 5 years of clinical acute care
  • Minimum of 2 years in quality management/performance improvement
  • Certification in Quality, Risk Management, or Patient Safety required, or willing to attain within two years of hire.
  • Experience with computer use. Must be able to use word processing and graphic programs. Additional experience with statistics or database programs preferred.
  • Knowledge of MIDAS software preferred.
  • Knowledge of the quality improvement principles used in Kettering Health Network preferred.
  • Excellent verbal, written, and presentation skills
  • Excellent communication skills with a wide variety of people
  • Excellent ability to achieve and maintain good report with physicians, healthcare leaders, work force, and patients.

Responsibilities

  • The Patient Safety Manager [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] will work closely with the Quality Medical Director, Patient Relations Representative, Diabetes Educator [Soin Medical Center only], and Director of Clinical Quality for Kettering Health System and other leaders and staff.
  • The Patient Safety Manager [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] will provide oversight and management for the Patient Relations service, which acts as the liaison between patients and the institution and between the institution and the public.
  • The Patient Safety Manager [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] will manage the Patient Relations Representative, who facilitates the grievance process for patients and families, in accordance with CMS (Center for Medicare and Medicaid Services) guidelines.
  • The Patient Safety Manager [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] will provide oversight of Risk and Patient Relations activities, including coordination with corporate Risk Management, negotiation of settlements, review and revision of policies and coordination of follow up related to risk exposure and liabilities.
  • The Patient Safety Manager [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] will accurately complete Grievance Reports and Safe Medical Device Reports through medical record review and communications with patients and their families and staff.
  • Collaborate with leadership and staff to promote a culture of patient safety and adherence to national best practice standards.
  • Review, follow –up, analyze and identify trends from adverse event reports.
  • Create and support educational programs in patient safety including education on use of Midas tool for event reporting.
  • Support, monitor, or implement safety initiatives as directed by Quality Director.
  • Help spread “lessons learned” from adverse events, as well as successful initiatives.
  • Foster a clinical environment of service excellence.
  • Provide management and support in the development, training, and implementation of [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] Patient Relations program.
  • Provide management and support in the development, training, and implementation of [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] Risk Management program.
  • Manage the daily operations of the Patient Relations and Risk Management programs at [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton].
  • Provide managerial oversight for the Patient Relations Representative at [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton].
  • Serve as knowledge expert in Patient Relations and Risk Management practices, processes, and Regulatory and Accreditation standards.
  • With the Director of Clinical Quality, Chief Quality Officer, and Corporate Risk Manager, establishes programs and policies.
  • Represents [Soin Medical Center ~ KH Greene Memorial ~ KH Hamilton] in standardization of Patient Relations and Risk Management for the Network.
  • Provide management and support in the development, training, and implementation of the [Soin Medical Center] Diabetes Education program.
  • Manage the daily operations of the Diabetes Education program [Soin Medical Center].
  • Provide managerial oversight for the Diabetes Educator [Soin Medical Center].
