About The Position

Michigan Institute of Urology is seeking a compassionate, organized, and dedicated Patient Navigator/ Prostate Cancer to join our state-of-the-art facility in Troy. If you are passionate about patient advocacy and helping families navigate the complexit ies of the healthcare journey, we want to hear from you. As a Patient Navigator, you are the bridge between clinical care and the patient experience. You will empower patients through knowledge and emotional support, ensuring they never feel alone while navigating their care pathway. The Patient Navigator is responsible for working directly with patients and their families to discuss any questions, complaints, comments, or suggestions. Advocates for patient in resolution of concerns. Empowers patients through knowledge and emotional support to independently navigate through the patient care pathway.

Requirements

  • High School Diploma or equivalent required.
  • Previous experience in patient advocacy, medical office administration, or healthcare navigation is preferred.
  • Knowledge of medical terminology and insurance coding.
  • Exceptional customer service and interpersonal communication skills.
  • Proficiency in Microsoft Office Suite and experience with EMR systems.
  • Ability to work independently, maintain strict HIPAA confidentiality, and show deep sensitivity toward the patient and family experience.
  • At least one year of experience in a medical office setting.
  • Minimum of two years experience in customer service relations.
  • Basic computer knowledge that includes Microsoft Office Suite. Knowledge in healthcare systems operations such as EMR.
  • Must communicate clearly and effectively in both a written and verbal format.
  • Knowledge of patient confidentiality, state laws, HIPAA regulations, and medical records procedures.
  • Ability to work independently and communicate appropriately with leader and teammates.
  • Understands and is sensitive to patient and family experiences.
  • Complies with HIPAA regulations for patient confidentiality.

  • Bachelor's degree in related field preferred.

Responsibilities

  • Guide patients through their healthcare experience, providing education on diagnoses and treatment options.
  • Identify and remove obstacles to treatment by coordinating referrals, scheduling appointments, and arranging necessary support services like transportation or translation.
  • Help patients understand insurance claims and work with them to create manageable payment plans when necessary.
  • Maintain accurate records by charting in the Electronic Medical Record (EMR) and tracking patient outcomes and metrics.
  • Connect families with appropriate support groups and age-appropriate resources.
  • Documents patient care services by charting in the patient’s electronic medical record. Updates information when changes have occurred, or new information is available.
  • Guides patients and families through their healthcare experience, answers questions, provides education, helps them to understand their diagnosis and treatment options.
  • Resolves patient problems and needs by utilizing team strategies.
  • Tracks and documents metrics and outcomes.
  • Identifies support groups appropriate for patients and families and coordinates scheduling of appointments.
  • Provides age and culturally appropriate information and resources during the diagnostic evaluation.
  • Removes obstacles to treatment by scheduling appointments, coordinating referrals, arranging transportation, translation, and other needed services.
  • Help patients navigate insurance claims and works to create payment plans when necessary.
  • Ensure that patients are fully informed about their legal rights.
  • Performs other position related duties as assigned.
