About The Position

The Department of Medicine, Division of Rheumatology, is seeking a dedicated and motivated Patient Navigator who is passionate about supporting patient care and ensuring a seamless experience for both patients and providers. This role is ideal for someone who enjoys problem-solving, communicating with patients, and working collaboratively in a clinical environment. The Patient Navigator (H) facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

Requirements

  • High School diploma or equivalent
  • Minimum 1 year of relevant work experience
  • Ability to process and handle confidential information with discretion.
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to maintain effective interpersonal relationships.
  • Ability to understand and follow instructions.
  • Any appropriate combination of relevant education, experience and/or certifications may be considered.

Responsibilities

  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
  • Ensures patients have access to insurance and social service programs for which they are eligible.
  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
  • Identifies and resolves real or perceived barriers to care.
  • Educates and counsels patients on issues related to their health and life situations.
  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.
  • Handle patient communications and contact healthcare facilities for medical records.
  • Address care gaps with the nurse navigator.
  • Prepares, tracks, and processes medical records and pathology specimens.
  • Reviews and file medical records for completeness; use tracking systems for accurate documentation.
  • Provides administrative support to the department by answering phones, address inquiries, and direct calls or take messages, Copy, fax, emailing, and mailing records while ensuring confidentiality.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.
  • Coordinate, submit, and follow up on 20-30 medication prior authorizations daily.
  • Provide phone support to the division as needed.
  • Confirm appointments and assist with patient navigation for the Division Chief.
  • Manage escalated patient concerns and ensure timely resolution.
  • Create and modify clinic templates.
  • Develop and maintain the Rheumatology inpatient schedule, update QGenda, and close clinics as required.
