SUMMARY: Under the supervision of the Practice Manager, the Patient Navigator is responsible for overseeing all emergency room and hospital admissions for our patients, along with monitoring abnormal mammograms to ensure all appropriate follow-up is completed. GENERAL DUTIES & RESPONSIBILITIES: Transition of Care Duties: Work is received through Outside Events bucket in Epic/OnBase/Patient Ping/Manual fax machine/Mail. Index all pertinent information received outside of OnBase to the patient’s medical record in Epic through OnBase. Document timely and accurately the pertinent information regarding the patient’s visit at another facility for Emergency Room or Inpatient stay, and route the information to the correct Departments in Epic. Responsible for distributing e-faxes to the correct folders in OnBase. Routes clinical issues to the appropriate clinical team. Handles all calls related from Outside Facilities and books appropriate follow-up appointments. Outreach to hospitals for needed information or missing information on discharge summaries Monitor the execution of transitional care services through ongoing quality assurance visits with referral sources. Meets quality metrics related to Transitions of Care. Attends Transition of Care meetings with DotHouse. Mammography Duties: Provides support with patient flow, including arranging additional appointments (biopsy, follow-up, call-back, etc.) with outside facilities Assists with support duties that include, but are not limited to, organizing and validating orders in the BMC database, sending patient result letters, and making confirmation calls that appointments were kept Works as an integral part of the team to assist with various operational duties such as helping with scheduling, ordering, and stocking supplies. Update Health maintenance for all six-month follow-up appointments, to ensure the patient was scheduled with an outside facility. Enter BIRAD data into the Mammography tracking system in Epic for Breast Imaging reports. Notify all patients who are overdue for their screening mammogram or other breast imaging exam on the reminder list via letters. Participates in staff meetings, takes the meeting minutes, continues education, and participates in ongoing workflow and process improvement initiatives. Demonstrates patient service excellence at all times. The ability to adapt to changes around the departmental needs, including but not limited to: offering assistance to other team members, floating departments, adjusting assignments, scheduling, etc. Perform other duties to support the clinical departments as assigned by the Practice Manager. Submits monthly environmental round report for review on a monthly basis.
