The Patient Navigator functions as the primary care coordinator for patients and collaborates with key clinical staff to ensure comprehensive care and treatment. In this role, you will serve as the Go-To contact for our patients receiving Oncology care, serving as a patient ally throughout the continuum of care. Patient Navigators serve as liaison to physicians and the department, prepping the patient with educational materials, logistics, and expectations. Manage treatment schedules, handle patient inquiries, gather medical records, and set consultations with the medical team. Demonstrate your caring & empathetic patient-first focus via your compassionate listening skills while troubleshooting solutions on behalf of patients. Location is our Columbia campus in Upper Manhattan/Washington Heights.
