Requirements

Bachelor's Degree in a Related Field of Study or equivalent years of experience required
Experience in healthcare, customer service, or a related field 0-1 year required
Experience working with patients 0-1 year required
Strong organizational and project management skills.
Strong oral and written communication skills
Excellent interpersonal and communication skills with the ability to connect with a broad spectrum of patient populations.
Proficiency in data management, reporting and analysis.

Nice To Haves

Proficiency with EPIC, Red Cap, and Microsoft Excel is desirable.
Spanish language proficiency is a plus.

Responsibilities

Works closely with patients and a multi-disciplinary care team.
Conducts needs assessments with patients to identify gaps and educates patients as appropriate.
Identifying, enrolling, and overseeing patients in various disease state programs, delivering updates on their performance, progress and strategic initiatives in a fast paced environment.
Maintains accurate and up-to-date patient care records and program metrics.
Identify trends and opportunities for program improvement.
Collaborate on initiatives to enhance holistic care delivery, incorporating feedback from patients, families, pharmacies, and care team members.
Foster and grow partnerships with external organizations and stakeholders to expand the program's reach and impact.
