Requirements

  • Bachelor's Degree in a Related Field of Study or equivalent years of experience required
  • Experience in healthcare, customer service, or a related field 0-1 year required
  • Experience working with patients 0-1 year required
  • Strong organizational and project management skills.
  • Strong oral and written communication skills
  • Excellent interpersonal and communication skills with the ability to connect with a broad spectrum of patient populations.
  • Proficiency in data management, reporting and analysis.

Nice To Haves

  • Proficiency with EPIC, Red Cap, and Microsoft Excel is desirable.
  • Spanish language proficiency is a plus.

Responsibilities

  • Works closely with patients and a multi-disciplinary care team.
  • Conducts needs assessments with patients to identify gaps and educates patients as appropriate.
  • Identifying, enrolling, and overseeing patients in various disease state programs, delivering updates on their performance, progress and strategic initiatives in a fast paced environment.
  • Maintains accurate and up-to-date patient care records and program metrics.
  • Identify trends and opportunities for program improvement.
  • Collaborate on initiatives to enhance holistic care delivery, incorporating feedback from patients, families, pharmacies, and care team members.
  • Foster and grow partnerships with external organizations and stakeholders to expand the program's reach and impact.
