About The Position

CommonSpirit Health is a large nonprofit Catholic healthcare organization dedicated to building a healthier future through integrated health services, delivering over 20 million patient encounters annually across 24 states. This Patient Logistics Coordinator position is based on-site at the Phoenix AZ Virtual Hospital. The role involves orchestrating seamless patient movement, optimizing bed utilization, coordinating transfers, and ensuring efficient patient flow to minimize delays and enhance operational efficiency. Key responsibilities include managing and prioritizing patient admissions, discharges, and transfers based on medical necessity and bed availability, and proactively communicating with clinical teams, discharge planners, and external facilities to resolve logistical challenges and ensure a smooth continuum of care. Success in this role requires strong clinical judgment, exceptional organizational skills, and the ability to make rapid, informed decisions in a dynamic environment.

Requirements

  • High School GED or High School Graduate.
  • One (1) year recent experience in an acute care setting OR Two (2) years recent experience in a health care setting.

Nice To Haves

  • Associate Of Arts or Associates Of Science degree.
  • Two (2+) years Medical Assistant or Nurse Assistant working in an acute care hospital setting.

Responsibilities

  • Orchestrate seamless patient movement, optimizing bed utilization, coordinating transfers, and ensuring efficient patient flow.
  • Minimize delays and enhance operational efficiency.
  • Manage and prioritize patient admissions, discharges, and transfers based on medical necessity and bed availability.
  • Proactively communicate with clinical teams, discharge planners, and external facilities to resolve logistical challenges and ensure a smooth continuum of care.
  • Work independently to facilitate the placement of patients as directed by the Patient Logistics Manager, Patient Logistics RN, and physician orders.
  • Facilitate the transfer of patients to the appropriate facility and unit.
  • Identify potential problems and bring them to the attention of the Supervisor, ANS/House Supervisor, and/or Leadership to prevent and/or resolve barriers to timely placement and/or transfer of patients throughout the acute care setting.
  • Communicate with referring and receiving facility staff to ensure timely placement of patients to the appropriate level of care and unit.
  • Document activities clearly and accurately to promote effective communication with members of the care team, including Transfer Center, Hospital EDs, Hospital Units, Leadership, sending and receiving facilities or other providers.
  • Use effective communication skills with colleagues, customers, physicians, and care team members, both internal and external to facilitate the timely placement of patients.
