About The Position

The Patient Flow Management Center (PFMC) and Center for Operational Resource Efficiency (C.O.R.E.) collaborate with patient care areas, support services, administration, and other departments to implement strategies that ensure seamless patient care, effective capacity management, efficient patient flow, improved access, and high-quality care. The primary goals of the PFMC and C.O.R.E. include reducing missed transfer opportunities, increasing targeted volumes, decreasing unnecessary length of stay (LOS), and ensuring that the right patient is placed in the right setting at the right time. The department is also responsible for defining the analytics necessary to predict and drive real-time decisions, tracking metrics, and reporting outcomes to foster an environment of continuous improvement. The Patient Flow Coordinator plays a crucial role in supporting the daily operations of the Patient Flow Management Center. This involves orienting, coaching, evaluating, and scheduling other Patient Flow Coordinators to ensure that patients are placed in a timely manner across the Division, operating 24 hours a day, 7 days a week. The coordinator monitors various indicators to expedite and prioritize challenges related to high occupancy and the over-utilization of resources. Additionally, the role requires coordination and communication with key personnel and departments, including Medical Staff, Nursing (Inpatient, ED, OR/PACU, Procedural Areas), Ancillary Departments (Care Management, Radiology, PT/OT), Environmental Services, Patient Transportation, and the Jefferson Transfer Center. The Patient Flow Coordinator also participates in bed coordination to ensure the most efficient bed utilization around the clock.

Responsibilities

  • Support the daily operations of the Patient Flow Management Center.
  • Orient, coach, evaluate and schedule the Patient Flow Coordinators.
  • Ensure patients are placed in a timely manner across the Division 24 hours per day/ 7 days per week.
  • Monitor tiles in order to expedite and prioritize challenges with high occupancy and over-utilization of resources.
  • Coordinate and communicate with key personnel and departments, including Medical Staff, Nursing (Inpatient, ED, OR/ PACU, Procedural Areas), Ancillary Departments (Care Management, Radiology, PT/OT), Environmental Services, Patient Transportation and the Jefferson Transfer Center.
  • Participate in bed coordination for the most efficient bed utilization 24 hours per day/ 7 days per week.
  • In real time, expedite and prioritize administrative issues to remove barriers to discharge and address delays in care.
  • Maintain patient throughout and continually evaluate bed placement progress according to the institution’s Patient Flow Plan.
  • Coordinate Direct Admissions, including urgent and scheduled, and contact patients and family members when the appropriate bed is available.
  • Participate in projects, data collection and other activities designed to enhance bed operations.
  • Identify and communicate quality and risk issues to the appropriate staff/ departments in a timely manner.
  • Monitor the EVS and Patient Transport Command Center to ensure that bed cleans and patient transports are assigned according to bed prioritization needs.
  • Provide leadership for Patient Flow Coordinators and work with PFMC colleagues to address patient flow issues.
  • Assist PFMC Leadership in defining administrative triggers and feedback mechanisms to inform operational team members, physician leaders, and administrative team to mitigate over capacity situations.
  • Assist PFMC Leadership in developing strategies to manage surge in patient volumes.
  • Assign beds to patients from all clinical departments, direct admissions, and external transfers based on defined workflow.
  • Liaise with PFMC staff and providers regarding administrative priorities and synchronization based on system needs.
  • Interact with co-workers, visitors, and other staff consistent with the values of Jefferson.
