About The Position

The Patient Data Analyst supports patient care and patient safety by ensuring the integrity of patient data across the health system including Master Patient Index, Vital Records, other demographic and insurance information, and released health information. Analyzes patient data and researches and resolves potential errors or omissions associated with the Master Patient Index, demographic and insurance information, and clinical data for targeted populations of patients. Maintains a database and reports identified issues to appropriate departments to support patient care, billing, and reimbursement. Responsible for fostering high levels of customer service in coordinating the collection and release of information to patients and their agents, regulatory agencies, the state of Georgia, and other HIM customers ensuring that they receive approPpriate, timely, and accurate health information. Demonstrates patience, sensitivity, and the ability to diffuse tense situations. Collects patient data both from the electronic medical record and by face-to-face interviews with patients and their agents both in HIM and in patient care settings. Coordinates patient generated HIPAA and other medical record amendment requests with the Director, Compliance, and appropriate caregivers. Enrolls HIM customers and their families in the patient portal. All activities will be performed in support of the strategy, vision, and values of Phoebe. Requirements

  • Associate's Degree in Health Information or High School Diploma or GED and 4 additional years of directly related experience (Required)
  • 3 years experience required; must include a combination of Healthcare data collection/monitoring or customer service in medical office/healthcare environment (Required)

  • Preferred Certifications/Licensures: Registered Health Information Technician (RHIT)

Responsibilities

  • Collects, analyzes, and provides information to patients and their agents, regulatory agencies, the state of Georgia, and other HIM customers
  • Handles all requests and inquiries for patient information.
  • Collects and disperses information with accountability to all regulatory requirements and policy and procedures.
  • Meets one-on-one with patients and their agents to facilitate the coordination of their health information, including enrollment in the patient portal, and completion of state mandated vital records.
  • Collects and disperses information on identified errors to appropriate individuals and department.
  • Demonstrates patience, sensitivity, and the ability to diffuse tense situations.
  • Maintain current knowledge and act as an expert resource related to hospital policy, state, and federal regulations including detailed understanding regarding (based on job function): Master Patient Index maintenance including patient identity and demographic data collection. Patient insurance verification and assignment. Collection of data and completion of Vital Records including birth certificates, paternity acknowledgements, and fetal death certificates. Regulations for Vital Records regarding unmarried, divorced, or separated mothers, same sex couples, adoptions (both public and private), surrogate parents, father is deceased or unavailable due to incarceration or military deployment, and minor parents. Medical record completion requirements including timeliness, what constitutes a complete medical record, and documentation requirements. State and federal regulations regarding release of information to patients, their agents and families, regulatory agencies, state of Georgia, and other customers of HIM. HIPAA as it pertains to confidentiality and medical records, including but not limited to release of information and amendment of medical records.
