The Patient Care Coordinator - Hospice is a high-level clerical position. This position requires the ability to coordinate care within the hospice program. The Patient Care Coordinator - Hospice may be requested to coordinate activities between inside/outside clinical resources. The Patient Care Coordinator - Hospice will report directly to the Clinical Manager. This position will ensure our organizational quality goals are met, and care remains compliant with all relative regulations, policies, and procedures. Day-to-day activities require assistance with scheduling changes, documentation of information, obtaining orders, updating medications, and assisting in the management of supplies, items and services related to the hospice benefit and the plan of care.
