About The Position

The Hospice Patient Care Coordinator (PCC) assists in the planning, implementation, and supervision of direct clinical services for hospice patients and families, ensuring high-quality care and compliance with organizational and regulatory standards. The PCC also supports staff development, monitors clinical operations, and may perform RN Case Manager duties as needed. Acts as the Program Director’s Designee when requested.

Requirements

  • Completion of an accredited Registered Nurse (RN) program.
  • Current RN license in the state of practice.
  • Must possess and maintain current CPR certification.
  • Minimum two years of nursing experience, including acute care and home care settings.
  • Strong clinical and patient care skills.
  • Excellent interpersonal and communication abilities.
  • Proficiency in electronic medical records (EMR) systems and office software.
  • Ability to work independently and as part of an interdisciplinary team.
  • Compassionate, empathetic, and dedicated to providing high-quality patient care.
  • Strong communication, problem-solving, and leadership skills.
  • Ability to lift up to 75 pounds and turn, move, and transport patients using proper body mechanics and lifting devices as needed.
  • Capacity to stand, walk, and sit for extended periods.
  • Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching.
  • Work is performed in a healthcare setting with potential exposure to infectious diseases, bodily fluids, and chemicals.
  • Must be able to work various shifts, including weekends, holidays as needed.
  • Participates in on-call duties.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Must be able to travel throughout the service area.

Nice To Haves

  • Bachelor’s degree in nursing preferred.
  • Certification in Hospice and Palliative Care Nursing (CHPN) preferred.
  • One year of hospice care experience preferred.

Responsibilities

  • Plan, coordinate, and supervise interdisciplinary team interventions according to patient plans of care and physician orders.
  • Assign patients/families to appropriate staff, ensuring optimal matches and addressing concerns as needed.
  • Perform or assist with assessments, care planning, and plan revisions based on changing patient conditions and needs.
  • Ensure that schedules are effectively managed to maintain appropriate 24/7 clinical coverage, meeting patient needs and emergency requirements in compliance with conditions of participation. Remain accessible and prepared to respond as necessary.
  • Ensure visits are conducted per the plan of care and physician orders.
  • Oversee the referral and admissions process, ensuring timely and accurate documentation.
  • Supervise nursing personnel and home health aides, conducting competency assessments and evaluations.
  • Collaborate with the interdisciplinary team to ensure seamless service delivery.
  • Monitor compliance with agency policies and procedures, completing audits and quality assurance reviews.
  • Maintain accurate and timely documentation of clinical records, including assessments, care plans, and progress notes.
  • Address potential areas of concern or risk, implementing proactive solutions.
  • Teach patients and caregivers self-care techniques, including medication, diet, and symptom management.
  • Provide health counseling and emotional support, promoting understanding and participation in care.
  • Conduct education and admission visits, offering compassionate, responsive care.
  • Provide leadership and support to staff, ensuring professional development and effective teamwork.
  • Identify training needs and participate in the creation and delivery of in-service programs.
  • Act as a resource for staff and a liaison between patients, families, and physicians.
  • Follow all HIPAA privacy regulations, maintaining confidentiality for residents and staff.
  • Attend all mandatory in-services, meetings, and training sessions.
  • Provide exceptional customer service through compassionate and empathetic care to patients and families.
