About The Position

Position Overview: The Case Manager is responsible for assisting families and individuals with identified needs. The person will assist all people seeking assistance from the Social Services Department. The Case Manager must maintain accurate files and perform all necessary data entry. The Case Manager also works from entry to exit with clients in our Pathway of Hope Program. Description of Position: Is responsible for conducting intakes of families and individuals needing assistance in all programs, offer assistance within the realm of the services our agency provides, obtain appropriate documentation needed for files, i.e. identification, consent and release forms, refer clients and participants to various community resources when we are unable to assist or provide the needed assistance, answer phone calls and messages, file client folders, conduct intakes, and perform other office duties as needed, accurately record information needed for reports, attend care management and department meetings as requested, interface with various social service agencies in the community through meetings and trainings. Inform clients and participants of the other services and programs the Corps offers and invite them to attend. Complete intake and assessments of potential Pathway of Hope participants. Maintain an organized case record that is updated following each meeting on each POH family into Client Tracks. Conduct client-led case management sessions either in home or in person to offer guidance and support with obtaining goals and discuss problems and progress associated with service plan. Provide referrals to other agencies for all programs. Accompany and, when needed, provide transportation to POH families to appointments if necessary. Complete monthly statistics and case summaries. Requirements

  • Excellent interpersonal and organizational skills
  • Minimum of two years of experience in residential or social service field
  • Ability to keep personal data confidential and adherence to all policies and procedures
  • Must possess excellent organizational skills.
  • Detail oriented, flexible, decisive, and able to work with minimal supervision
  • Ability to thrive in a collaborative team environment but also able to work independently
  • Child Protection Clearances PA Act 153 requires that all prospective employees complete the following clearance and background checks prior to employment: PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record/Fingerprint Check.
  • Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check along with renewals every five years.
  • Must have an understanding of and support for the Mission of The Salvation Army.

Nice To Haves

  • Bachelor’s degree in Social Work, Psychology, Sociology, or Education preferred
