About The Position

The Palliative Care Licensed Practical Nurse (LPN) provides high-quality nursing care and clinical coordination under the direction of an RN or provider. This role supports symptom management, patient and caregiver education, care planning, admissions coordination, and continuity of care across the palliative care continuum. The LPN also serves as a knowledgeable resource for patients, families, and referral partners regarding EverHeart Hospice’s Palliative Care services.

Requirements

  • Current Ohio RN license
  • Minimum 3 years hospice experience (Med/Surg, Oncology, or Community Health also considered)
  • Valid driver’s license and reliable transportation
  • Strong communication and time management skills
  • Ability to work independently

  • Healthcare sales and/or marketing experience preferred but not required.

Responsibilities

  • Provide skilled nursing care in accordance with individualized care plans and palliative care best practices
  • Monitor, assess, and report changes in patient condition, symptoms, and response to treatment
  • Educate patients and caregivers on disease progression, treatment options, and goals of care
  • Collaborate closely with physicians, Nurse Practitioners, RNs, Admissions, and interdisciplinary team members
  • Assist with referral coordination, care planning consultations, and service matching
  • Support pre-admission and admission processes, including paperwork, physician orders, EMR documentation, and coordination of medications and DME
  • Maintain accurate, timely documentation in the electronic medical record (EMR)
  • Conduct follow-up phone calls and visits as directed by the Nurse Practitioner
  • Assist with coding and documentation to support timely and accurate claims processing
  • Serve as backup support for hospice and palliative admissions and intake functions
  • Travel to patient homes and partner facilities as needed
  • Follow up with and monitor patients in the Palliative Care Program and the In Touch Program.
  • Perform other duties as assigned to support patient care and team operations
