As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center. General Flexible role (full time and part-time options available) Established team of 1 Medical Director and 4 Advanced Practitioners Established practice serving the community Experienced staff, both clinical and non-clinical, who know the patients and the community Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination

Requirements

  • Nurse Practitioner or Physician Assistant
  • Ability to work as a team member
  • Hold an unrestricted medical license in the state of Iowa
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice

Nice To Haves

  • Experience in post-acute and/or long term care facility preferred
  • Palliative Care certification preferred

Responsibilities

  • Full & part-time roles
  • Clinic hours: Monday-Friday 8am-5pm
  • 4 day work week
  • No nights, weekends, or holidays
  • Palliative Care services provided in patient's homes, clinic and/or nursing home setting
  • Average of 8-10 patients per day; no nights or weekends
  • Shared call (telephone only) amongst the providers. Currently 1:17 – Triaged through MyNurse
  • Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care
  • Patient-focused, friendly, and willing to collaborate in a team environment
  • Excellent communication (written and verbal), analytical and problem-solving skills
