About The Position

The PACE Center Manager serves as an on-site operational leader and clinical support resource within the PACE center, ensuring the delivery of high quality, coordinated care for participants. This role is primarily responsible for overseeing daily center operations, supporting clinical workflows, and coordinating home health scheduling needs. The Center Manager functions in a supportive clinical capacity by gathering, monitoring, and communicating participant condition changes to RN Care Coordinators and providers, while ensuring timely follow through on care plans and services. Valir PACE provides integrated medical and social services to our most vulnerable seniors, helping to unlock their full potential. Requirements

Current Licensed Practical Nurse license in good standing
Current CPR Certification
Minimum two years of clinical experience

Nice To Haves

Prior leadership or supervisory experience preferred
Experience in geriatric care, primary care, or PACE preferred
