About The Position

The South Florida AIDS Network/Outreach workers are responsible for collaborating with service providers, including medical case managers, to locate individuals who have fallen out of medical care or medical case management and facilitate their return to care. Referrals for outreach workers are typically made by medical case managers or pharmacy staff after unsuccessful attempts to contact clients via phone and mail.

Requirements

  • Needs reliable transportation, car insurance and a valid driver's license.
  • Understands and practices confidentiality.
  • Generally requires 0 to 3 years of related experience.
  • High School diploma is required.
  • Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines.
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to understand and follow instructions.
  • Ability to exercise sound and independent judgment.
  • Knowledge and skill in use of job appropriate technology and software applications.
  • All staff providing Peer services must be certified through the State of Florida's Department of Health HIV/AIDS 104 course.

Responsibilities

  • Accompany newly diagnosed, lost to care, or otherwise unconnected HIV+ clients to initial physician appointments, medical case management appointments, and/or intake at a residential substance abuse facility of the client's choice to reconnect them to care or enroll them in service.
  • Make home visits to meet with clients for the purpose of connecting them to care.
  • Conduct brief intakes for new clients and enter data into the system outreach registration screen.
  • Act on approved referrals and review data in the system for existing clients who are lost to care or are at risk of falling out of care.
  • Assist clients, as necessary, to obtain required documents for entry into the service system.
  • Obtain all required consents for the outreach worker to access client-related information in the Ryan White Program.
  • Provide education on available care and treatment options and services for HIV+ individuals to empower and enable them to access existing HIV/AIDS service programs, including Counseling & Testing sites.
  • Provide out-stationed linkage and coordination to care services at key points of entry, such as counseling and testing facilities and other facilities with a high percentage of HIV+ clients, as identified by the counseling and testing facility and verified by the Ryan White Part A Program.
  • Coordinate and participate in planned outreach/testing events in cooperation with the Miami-Dade County Health Department.

  • Perform all other related job duties as assigned.
