About The Position

As an Outreach Worker, you greatly assist community members by providing meaningful interactions and connections to a variety of community-based resources and programs. STATUS: Part time (24 hrs.) LOCATION: Newark-Wayne Community Hospital - 1200 Driving Park Avenue Newark, NY 14513 DEPARTMENT: Hospital Operations SCHEDULE: Monday - Friday 9:00 am - 3:00 pm ATTRIBUTES Two (2) years of experience required Strong computer and interpersonal and communication skills Superior customer service and empathetic attitude RESPONSIBILITIES Patient Care. Demonstrate knowledge of community resources and how to access them; complete initial assessment for new, eligible participants and collaborate with other care providers regarding participant needs; provide referrals for participants and their families; make home visits as needed; advocate for patients to ensure adequate housing, treatment, benefits and community services; develop patient treatment plan Documentation. Maintain accurate and complete record documentation and data collection; maintain patient related demographic information Program Development. Provide oversight in the development of newsletters, brochures, flyers and informational talks; coordinate and attend community activities that promote the Healthy Start Center; identify and coordinate relevant staff education needs; develop policies and procedures

Requirements

  • Two (2) years of experience required
  • Strong computer and interpersonal and communication skills
  • Superior customer service and empathetic attitude
  • NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)New York State Department of Motor Vehicles (NYSDMV)

Responsibilities
