POSITION SUMMARY The Outreach Worker identifies older adults (age 55+) who have a substance use or mental health problem for which they would benefit from treatment. The Outreach Worker provides engagement, assessment and case management services, and education through community-based outreach and in-service training. These services are offered in older adults’ homes, senior centers, senior housing, and other community and social service settings. The goal is to support older adults in engaging in services that will allow them to remain integrated in the community in the least restrictive setting. Rate $20.00

Requirements

  • High School diploma/GED and a minimum of five years of experience with similar community education and outreach.
  • Working knowledge of substance abuse is required.
  • Valid Drivers License is required.
  • Safely operates vehicle in the community, based on program needs.

Nice To Haves

  • Bachelors degree and some experience preferred.
  • Bi-lingual in Spanish is preferred.

Responsibilities

  • Provides in-service training sessions to traditional and non-traditional settings within the region.
  • Makes contact with older adults identified as in need of behavioral health treatment in Central and North Central Connecticut (Greater Hartford, New Britain, Plainville, Bristol, Manchester, Enfield areas).
  • Provides outreach and engagement for older adults employing Motivational Interviewing.
  • Assess and provide brief intervention and referral for older adults employing the SBIRT model (Screening, Brief Intervention, and Referral to Treatment). Continue to provide case management services for clients ensuring their behavioral health and basic needs are met.
  • Makes formal treatment referrals for older adults and coordinates services if client leaves treatment for non-compliance or AMA.
  • Collaborates with other providers within the region and develops linkages with organizations that provide services to older adults.
  • Maintains records of program activities for reporting and tracking purposes. Creates demographic/referral sheet for each identified client being referred for formal treatment (To be reported to DMHAS).
  • Attends monthly or quarterly staff meetings and assigned community meetings.
  • Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
