About The Position

The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. The Bureau of Chronic Disease Prevention (BCDP) works to eliminate racial inequities and reduce overall premature mortality by tackling big tobacco, big salt, big sugar, and other commercial determinants of health. As part of prevention and wellness promotion, BCDP employs evidence-based policies, programs, communications, and research to advance its objectives, as well as spearheads and evaluates innovative projects in order to develop new approaches to addressing chronic disease. The Bureau supports work spanning Tobacco Policy and Programs, Healthy Eating Initiatives, Active Design, Public Health Detailing, Cancer Prevention and Control Program and the Hypertension Initiative. The Public Health Detailing (PHD) Program within the Clinical and Scientific Affairs Unit is an educational outreach program that works directly with health care providers and staff to improve patient care related to key public health challenges. PHD is organized around specific clinical topics, chosen largely because of their anticipated impact on morbidity and mortality. The Program is seeking to hire an Outreach Representative to conduct in-person outreach visits to health care providers' offices to promote clinical systems improvement and chronic disease management and offer supporting materials to assist with implementation. Please note A valid NYS Driver's License is required.

Requirements

  • A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
  • High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
  • Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
  • A valid NYS Driver's License is required.

Responsibilities

  • Conduct educational outreach visits to promote specific preventive health interventions to targeted health care providers and associated staff within assigned geographic areas within in New York City.
  • Serve as a resource to health care practices within assigned territory and fulfill requests as needed.
  • Collect and report results of outreach visits and research health care provider universe to maintain a comprehensive listing of health care practices in assigned territory.
  • Enter both process and outcome data acquired from detailing visits of both health care providers and staff in the various required reports in a timely manner.
  • Participate in regular trainings and provide best practices and feedback on effective outreach and systems change tactics.
  • Be an active participant in campaign training development by preparing training modules on the detailing method specific to communication and selling skills, for both internal and external contracted representations and be a resource of technical assistance for all reps.
  • Assist with maintaining inventory of current campaign materials and inform outreach manager when materials are running low.
  • Work collaboratively with Public Health Detailing central office, Bureaus of Neighborhood Health and other agency programs on projects associated with health care providers and campaign-specific activities.
  • Hold a valid NYS Drivers' License and has established a good driving record.
  • Other duties as requested by Outreach Manager.
