Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. Job Summary The Outreach Navigator’s primary role is to enhance the likelihood of success, identify and reduce barriers for SMI clients and/or eligible MHBG individuals and connect them with needed services and resources, including COVID-19 Personal Protective Equipment (PPE) supplies and education. The Outreach Navigator will work with and outreach SMI individuals in need of additional support and engagement case management, and ongoing wrap around supportive services. Services include COVID-19 PPE supplies, and COVID-19 education and resources. The Outreach Navigator will provide services to individuals pre/post incarceration, as well as individuals with a recent Crisis Mobile Team (CMT) activation. The Outreach Navigator maps out the most appropriate next steps for individuals in need of assistance with service access. The Outreach Navigator is part of a multi-disciplinary team made up of internal and external stakeholders. The Outreach Navigator will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.

Requirements

  • Highschool diploma or GED is required.
  • Must have lived experience in recovery from mental health and/or substance use disorders, homelessness, family members and veterans.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
  • Current AZ Driver’s License (Valid and in good standing).
  • Clear 39-month Motor Vehicle Report
  • Fingerprint Clearance Card (Program Specific)
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101 33 and CBI Clinical Policies and Procedures preferred.

Nice To Haves

  • Associates Degree (or higher) in a field related to Behavioral Health is preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101 33 and CBI Clinical Policies and Procedures preferred.
