About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency with programs across Arizona, Oklahoma, and the District of Columbia. Established in 1982 as a non-profit, CBI's core mission is "maintaining the dignity of human life." The District of Columbia Stabilization Center (DCSC), operated by CBI, provides immediate, 24/7, no-cost, low-barrier access to Crisis SUD services for individuals 18 and older, regardless of gender, insurance, or citizenship status. The DCSC offers a person-centered, recovery-oriented alternative to emergency departments or law enforcement for those experiencing an SUD crisis, ensuring integrated and comprehensive care in a supportive clinical and recovery-oriented environment. The Outreach Navigators will conduct comprehensive outreach in Wards 7 and 8 in Washington D.C., leveraging their lived experience with opioid use, mental health, behavioral health, and/or homelessness. This role is crucial for enhancing client success, identifying and reducing barriers, and connecting individuals with necessary services and resources. The Outreach Navigator will engage with and outreach to individuals requiring additional support, case management, and ongoing wrap-around supportive services, guiding them through the process of accessing assistance. This position is an integral part of a multi-disciplinary team, collaborating with both internal and external stakeholders.

Requirements

  • Highschool diploma or GED is required.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.

Nice To Haves

  • Associates Degree (or higher) in a field related to Behavioral Health is preferred.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.

Responsibilities

  • Provide comprehensive outreach to Wards 7 and 8 in Washington D.C.
  • Utilize lived experience in opioid use, mental health, behavioral health, and/or homelessness to enhance the likelihood of client success.
  • Identify and reduce barriers for clients.
  • Connect individuals with needed services and resources.
  • Work with and outreach individuals in need of additional support, case management, and ongoing wrap around supportive services.
  • Map out the most appropriate next steps for individuals in need of assistance with service access.
