About The Position

The Outreach Liaison of the San Diego County Black Infant Health Program is responsible for providing outreach engagement services to recruit clients into the program. The Outreach Liaison participates in, and supports, the planning, development, and implementation and evaluation services in accordance with the contractual and departmental requirements and guidelines.

Requirements

  • Basic administrative skills including scheduling, documentation, report writing, computer skills, and MS Office skills;

  • Local and community services and familiarity with particular communities in which services will be provided;
  • Understanding and respect of the values and beliefs of African American women and the African American community;
  • Maintain good interpersonal interactions with clients and coworkers as a member of multi-disciplinary community health team with a diverse multi-cultural population;
  • Maintain exceptional organizational skills, attention to detail, time management skills, and strong motivation to meet deadlines and achieve goals;
  • Communicate effectively orally and in writing;
  • Sensitivity to and understanding of the unique challenges of potentially high risk pregnant and/ or parenting African American women;
  • Demonstrate professional and positive role modeling;
  • Exercise appropriate judgment and discretion in answering questions, releasing information, and maintain confidential information;
  • Maintain composure in situations which require a high degree of sensitivity, tact and diplomacy;
  • Strong interpersonal and presentation skills;
  • Demonstrates cultural competency and ability to operate in a culturally affirming manner;
  • Bachelor’s degree in Social Work, Public Health; Health Education, or Social Science or health related field required;
  • Must attend CDPH/ MCAH sponsored training;
  • One (1) year of experience in working in a community based or health care setting providing direct services to women and African Americans required;
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation, proof of liability and property damage insurance on transportation used is required;
  • Licensed driver for a minimum of three (3) years with no more than two (2) violations and accidents within the last three (3) years; no vehicle related suspensions/ reinstatements, DUI’s, reckless or felony driving within the last seven (7) years;
  • A valid California driver's license with current insurance.
  • Current CPR certification is required or to be obtained within 90 days of employment;
  • TB test required upon hire and must be maintained every four (4) years.

Responsibilities

  • Participates in the development, implementation, and maintenance of a strategic recruitment plan for the program;
  • Develops and maintains relationships with medical and community service providers who are the primary referral sources to the program, including developing partnership agreements to assist in providing referrals to the program and conducting outreach activities on a regular basis;
  • Takes the lead in setting up and staffing BIH table at community health and/or resource fairs;
  • Provides BIH related presentations to community partners, agencies and individuals to help increase program referrals and awareness of BIH program, services and goals;
  • Provides the initial contact for new referrals to assess for eligibility and interest in BIH program participation;
  • Works with the program’s Mental Health Specialist to schedule BIH enrollments;
  • Responsible for ensuring that the program’s outreach goals are met for each program year;
  • Completes all required documentation accurately, in a timely manner and thoroughly in accordance with departmental standards. Assists in preparing reports as required;
  • May help to recruit program participants for social media opportunities/interviews per State and County BIH guidelines;
  • May work with NHA and other BIH program staff to ensure regular and relevant postings to the agency’s social media platforms;
  • Performs other duties as assigned.
