Role Scope : This is an opportunity for a mission critical leader. The Outreach & Enrollment Supervisor is responsible for closely developing and managing a team of Habitat Health Outreach Specialists, Enrollment Specialists, and Intake Coordinator, as well as key community partners. Critical job responsibilities will include ensuring prospective participant enrollment success, managing the team’s individual and overall performance against targets on a monthly and yearly basis, and supporting the sourcing and maintaining of local outreach channels for prospective participants. This supervisor will be responsible for leading new participant growth efforts for the Sacramento Center location, including ensuring future prospects have a comprehensive financial and functional assessment and meet all compliance requirements by DHCS and CMS to enroll in our PACE program. The Outreach & Enrollment Supervisor will also be part of the Sacramento Center’s leadership team and work cross-functionally to ensure that new participants thrive at our centers and Habitat Health continues to be a great place to work for our team members. The Outreach & Enrollment Supervisor is expected to review metrics daily, support growth initiatives, coach staff members and ensure the Outreach and Enrollment team operates smoothly and successfully in collaboration with the Sacramento Center’s Executive Director and Habitat Health’s Growth leadership.

Requirements

  • Minimum of 3 years in healthcare sales, outreach, intake, enrollment, social services, healthcare navigation, case management, or community engagement
  • A minimum of one year experience working with the frail or elderly
  • Strong computer skills
  • Ability to create and analyze excel spreadsheets
  • Experience managing through a CRM system
  • Experience utilizing and navigating EMR systems
  • Analytical approach toward sales data & performance
  • Comfort with personal sales quota in addition to management responsibilities
  • Excellent relationship and people management skills with the ability to motivate a team
  • Strong focus on data and analytics to drive targeted activities and return on growth efforts
  • Ability to manage competing priorities while maintaining a positive attitude
  • Inclination to work in a results-oriented role
  • Experience working cross-functionally to streamline workflows
  • Ability to navigate complex situations with families, caregivers, and external partners
  • Willing to work weekends as necessary, particularly for community events
  • Proof of medical clearance for communicable diseases, including a TB test
  • Proof of all immunizations is up to date
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law

Nice To Haves

  • Prior experience in a sales management role or other comparable management position
  • Familiarity with the PACE program or other elderly care organizations
  • Bilingual: Spanish/Mandarin/Cantonese preferred.
  • Deep understanding of the local Sacramento health care community

Responsibilities

  • Manage and develop a team of 5-10 quota-carrying Outreach and Enrollment Specialists, as well as Intake Coordinators, across 1-2 PACE centers
  • Carry and deliver against targets for monthly team contribution to participant growth
  • Develop both the long-term vision and strategy for the team while managing day-to-day Outreach & Enrollment team operations
  • Provide an unmatched experience for participants and prospects
  • Troubleshoot bottlenecks and proactively remove operational obstacles
  • Provide real-time guidance on complex cases, eligibility considerations, family dynamics, and barriers to enrollment
  • Develop and implement strategies to identify and strengthen referral pathways with community organizations, providers, and partners in the greater Sacramento area
  • Support Outreach Specialists and the Growth leadership team on local marketing events to engage key stakeholders in the greater Sacramento area
  • Be an ambassador in the local community to help engage prospects – building relationships with other healthcare providers, social / community organizations, and marketing partners
  • Identify and share best practices across the organization
  • Provide skill training, mentoring, and quality checks on participant and prospect engagement
  • Conduct auditing efforts to ensure quality assurance and to meet DHCS and CMS guidelines
  • Recruit, retain, and nurture team of Outreach & Enrollment Specialists with high potential and strong performance history
  • Serve as a cross-functional leader by collaborating across the organization, including supporting key strategic initiatives related to center and clinical operations, participant experience and journey, and health plan operations as they intersect with the Outreach & Enrollment team
  • Represent the Sacramento Outreach & Enrollment team at both the Growth leadership and Sacramento Center leadership levels, facilitating cross-functional engagement to align on and drive progress against key initiatives
