About The Position

Provides personal care and homemaker services for hospice terminally ill patients and their families. Care is provided per the hospice interdisciplinary plan of care to patient in their homes or facilities where they live, independently under indirect nursing supervision. Personal care services include any services related to activities of daily living including bathing, dressing, ambulation/transfer, feeding, incontinence care, positioning and other activities that support patient/family quality of life. Responsible to the primary nurse for carrying out the duties assigned in order to increase the level of comfort and to maintain personal hygiene and safe home environment. Provides homemaking services such as straightening the patient area, washing the patient's clothes and cooking simple meals. Provides temporary respite and relief from total care responsibilities for the primary caregiver and family. Functions as a member of the Hospice interdisciplinary team.

Requirements

  • High School Diploma required
  • 1 year Patient Care experience in a health care setting required
  • Basic Life Support (BLS)
  • Cardiopulmonary Resuscitation (CPR)
  • Certified Nursing Assistant (CNA)

Responsibilities

  • Document care provided in Suncoast Solutions EMR software.
  • Accurately submit mileage documentation.
  • Clearly and accurately document designated processes, policies, products, service offerings, etc.
  • Ensure that documentation is tailored to expected readers / users.
  • Use correct terminology.
  • Conform to required style and format.
  • Treat patients and families with compassion and implement plan of care.
  • Base care decisions on plan of care.
  • Monitor and document patient response.
  • Establish rapport with clients.
  • Adapt communication style, listen and respond to patient needs.
  • Recognize age-specific responses to treatment.
  • Provide effective interventions.
  • Respect patients confidentiality and privacy.
  • Support social, spiritual, and cultural needs of patients and families.
  • Manage difficult or emotional patient situations.
  • Treat patients and families with compassion and implement the hospice interdisciplinary plan of care.
  • Manage and deliver patient supplies to homes.
  • Create successful interpersonal relationships with patients and families.
  • Communicate clinical information to patients, family caregivers, and other staff (read, write, and verbally report).
  • Observe, report, and document patient status and the care or services furnished.
  • Perform appropriate and safe techniques in personal hygiene and grooming tasks, including bed bath, sponge, tub, shower bath, hair shampoo, nail and skin care, oral hygiene, toileting and elimination.
  • Perform safe transfer techniques and ambulation, normal range of motion and positioning.
  • Ask patients/families what is important to them on an ongoing basis, what brings them peace, comfort, joy.
  • Read and record vital signs.
  • Implement infection control procedures.
  • Maintain a safe environment.
  • Ensure adequate nutrition and hydration.
  • Recognize emergencies and procedures for reporting same.
  • Respect the physical, emotional, and developmental needs of and ways to work with the populations served by the hospice, including the need for respect for the patient, his or her privacy, and his or her property.
  • Participate in IDT care plan collaboration meetings.
  • Offer quality of life closure activities supporting patients/families experiences.
  • Recognize, report and collaborate with IDT regarding suspected abuse, neglect or mishandling of patients.
  • Provide respite care as assigned.
  • Assist with providing respite care for patients.
  • Safely provide personal care to clients based on the plan of care.
  • Notify primary nurse, on-call nurse or Patient Care Coordinator of patient care issues.
  • Provide homemaker services such as straightening patient area, assisting with patient laundry, and cooking simple meals.
  • Adhere to the hospital and departmental attendance and punctuality guidelines.
  • Perform all job responsibilities in alignment with the core values, mission and vision of the organization.
  • Perform other duties as required and complete all job functions as per departmental policies and procedures.
  • Maintain current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).
  • Attend staff meetings and complete mandatory in-services and requirements and competency evaluations on time.
  • Demonstrate competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
  • For non-clinical areas, attend training and demonstrate usage of age-specific customer service skills.
  • Wear protective clothing and equipment as appropriate.
