About The Position

UPMC’s Adult Ambulatory Behavioral Health Services is looking for an Outpatient Care Coordinator to join our team! The Adult Ambulatory Behavioral Health Service line consists of an Outpatient department that offers individual and group therapy and medication management on an outpatient level, our Intensive Outpatient Programs and Partial Hospitalization Programs, as well as our Behavioral Health program for Pregnant and Postpartum Women. As an Outpatient Care Coordinator (OCC) you will work as team along with other OCCs to assist and support our clinical teams including working collaboratively with our Clinicians, Residents, and Attendings in all Adult Services programs. This position will work day time hours with a hybrid schedule requiring work onsite in Oakland as well as remote from home.

Requirements

  • Bachelor’s Degree in human services or Business related fields required.

  • Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Act 31 Child Abuse Reporting with renewal

  • Act 73 FBI Clearance with renewal

Nice To Haves

  • Preferred degree in Psychology, Social Work, or Education.
  • Completion of a human services related internship/practicum or 1 year of relevant human services experience preferred.

Responsibilities

  • Conducts comprehensive assessments to identify consumer needs across clinical, social, and support areas.
  • Applies Recovery Principles to promote self‑management.
  • Collaborates with treatment team to maintain accurate caseload engagement.
  • Maintains complete and compliant case records, including intake and commitment paperwork.
  • Completes referrals, notes, and required documentation promptly.
  • Assists with involuntary commitment processes and emergency coordination.
  • Manages hospital discharge documentation in the electronic record.
  • Provides timely reports and required data.
  • Facilitates access to services through referrals, follow‑up, and activity tracking.
  • Collects key consumer information to support continuity of care.
  • Educates consumers on available agency and community resources.
  • Coordinates with treatment teams and community supports to ensure continuity of care.
  • Tracks and monitors intake data in manual and electronic systems.
  • Uses DSM to develop provisional diagnoses.
  • Manages administrative steps for new referrals and consults clinical staff as needed.
  • Advocates for consumer and family involvement in the recovery process.
  • Maintains knowledge of insurance and referral requirements.
  • Participates in treatment team meetings and collaborates on care plans.
  • Monitors engagement and addresses barriers to participation.
