Job Summary: Provides supportive counseling/social work services to patients and families, acts as a liaison with other community resources. Assists staff and volunteers in understanding significant; social and emotional factors affecting a patient and family during advanced illness. Responsibilities

  • Interprets program/services for patients and families, reviews referrals, and initiates psychosocial assessments to identify patient/family needs, resources, and acceptance.
  • Participates in the Interdisciplinary Group (IDG) meetings assisting in the development of a Comprehensive plan of care; communicates and interprets patient/family needs and problems to the IDG.
  • Maintains ongoing, supportive counseling with patients/families, assisting them with difficulties, and meeting social and emotional needs.
  • Makes appropriate referrals to community agencies and/or community resources. Also advises staff/volunteers regarding services available to patients and families.
  • Advises and assists in third-party reimbursement.
  • Assists with program planning, development and evaluation.
  • Serves as liaison with community agencies, churches, the medical community.
  • Maintains established departmental policies and procedures, objectives, performance improvement, safety, environmental and infection control standards.
  • Implements patient care delivery model to meet the needs of the patient population served, with consideration to age specific needs for optimal outcomes.
  • Strives continually to improve career by broadening knowledge through attendance at in-service meetings and by individual research and reading. Maintains professional growth and development through seminars, workshops and professional affiliations.
  • Serves as the Survivorship Program Coordinator and Psychosocial Services Coordinator for the Cancer Committee to meet Commission on Cancer standards.
  • Identifies gaps in psychosocial resources and initiates the development of solutions, including sourcing funding, drafting policies and procedures, collecting and presenting data, and implementing ongoing program improvements.
  • Assists with and participates with community events sponsored by Good Samaritan Hospital/Cancer Center.
  • Assists to maintain a positive working environment by working together as a team member.
  • Makes scheduled visits and assures coordination with the Patient Coordinator.
  • Participates in scheduled meetings.
  • Performs other duties as assigned by department Director or designee.
