About The Position

Freedom Square of Seminole is looking for a Social Services Coordinator to join our team! Come experience the Freedom Square of Seminole difference! We are an elegant, Upscale Senior Living Community, nestled in a 15-acre certified wildlife habitat. As the Social Services Coordinator you will be responsible that medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with policies and procedures and current Federal and State guidelines and regulations. This position serves as the licensed Social Worker for the Health Center.

Requirements

  • Bachelor's degree in social work, Gerontology or related field. Must be a licensed Social Worker (in current state of residence)
  • At Least 2 Years experience, in a healthcare setting (preferably Long-Term Care) working directly with residents and families.

Responsibilities

  • Develops a comprehensive social history and assessment of the resident that includes problems and strengths, special needs and preferences (social, religious and /or cultural) and the implications for the care plan.
  • Assists with the coordination of Health Center admissions and assessments.
  • Assists the resident/family with the transition to the new environment, orients the resident/family to the facility, services, limitations, and resident's rights.
  • Participates in the development of the individuals care plan including the goals and approaches for the Social Services component.
  • Attends scheduled care planning conferences, addressing particular issues of schedule, treatment and environment, and encourages residents and family (as appropriate) to participate in care planning conferences.
  • Ensure that the Advances Directives are complete and maintained and placed in resident charts.
  • Completes all mandatory documentation in a timely (determined) manner as established by applicable regulations.
  • Maintains confidentiality of all admissions information and resident records following HIPPA and the community's Code of Ethics.
  • Coordinates discharges/transfers to ensure the appropriateness and continiuity od care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
