Trillium Woods is recruiting for a hospitality focused Community Social Worker to join our team! This position is responsible for full-scope, hands on production cooking in the Community Culinary Services Department. 

Requirements

Degree in Social Work, Gerontology, or a related field
2–3 years of experience in long-term care, transitional care, memory care, or equivalent healthcare settings preferred
Proficiency in technology, including Microsoft Office Suite
Strong interpersonal and communication skill

Nice To Haves

Licensure as a Social Worker in Minnesota is preferred

Responsibilities

Support coordination of resident care and services across the community’s continuum of care
Serve as a liaison between residents, families, physicians, and community staff to ensure clear communication and coordination of care
Assist with resident admissions, discharges, and transitions
Maintain accurate, current, and confidential social service documentation and resident records
Assist residents in accessing internal and external resources
Participate in care conferences and interdisciplinary meetings to support care coordination
Support Social Services in ensuring advanced directives are completed, updated, and properly maintained in resident records
Meet with prospective residents and perform the Memory Health Assessment
Support resident wellbeing by assisting with wellness initiatives
Coordinate short-term and long-term stays within higher levels of care
