SOCIAL SERVICES COORDINATOR JOB DESCRIPTION A. CLINICAL/ADMINISTRATIVE: Ensures the residents' psychosocial and concrete needs are identified and met in accordance with Federal, State, and Company requirements. Orients residents to the center. Discharge planning. Implements and updates Resident Care Plan and Social History. Maintains records of outside referral. Assists in coordinating residents' financial affairs. Communicates needs and Care Plans to resident, family, responsible party, and appropriate staff. Participates in meetings, in-services, and training as required. Meets with consultants as required. Organizes and facilitates Family Group. Organizes and conducts New Admissions Group. B. CONSUMER SERVICE: Presents professional image to consumers through dress, behavior, and speech. Adheres to Company standards for resolving consumer concerns. Ensures that all consumer and patient rights are protected.
