About The Position

At Brandermill Woods, we are dedicated to providing compassionate, resident-centered care within our continuing care retirement community. We are currently seeking a Social Services Coordinator to support the emotional, psychosocial, and transitional needs of residents in our healthcare community, including long-term care are skilled nursing. This is an ideal opportunity for someone who enjoys meaningful, relationship-driven work and wants to play an active role in resident care planning and interdisciplinary collaboration. This position offers flexibility while maintaining a consistent weekday schedule. You will work closely with residents, families, and clinical teams to help ensure each resident's voice is heard and their needs are met.

Requirements

Bachelor degree in Social Work, Psychology, Sociology, or related Human Services field preferred
Experience in long-term care, skilled nursing, or healthcare setting strongly preferred
Demonstrated initiative and ability to work independently within a team environment
Strong communication, organization, or interpersonal skills

Nice To Haves

Experience with PointClickCare (PCC) or similar EMR system is a plus

Responsibilities

Support residents and families through admissions, transitions, and ongoing care needs
Assist with care plan development and participate in interdisciplinary care plan meetings, particularly for long-term care residents
Conduct social histories and psychosocial assessments
Provide emotional support and resource coordination for residents and families
Collaborate with nursing, therapy, and leadership teams to ensure holistic care
Assist with discharge planning and continuity of care
