About the Role: We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Broomfield, CO. You’ll work one-on-one with patients to help them regain independence, improve function, and enhance quality of life.
Requirements
- A valid Occupational Therapist license in Colorado
- Proven experience in home health or a related field
- Excellent communication and interpersonal skills
- Proficiency in electronic medical record documentation
- A flexible and adaptive approach to meet the diverse needs of your clients
Responsibilities
- Develop and implement individualized treatment plans
- Conduct evaluations using standardized assessments and clinical expertise
- Deliver functional training in ADLs, mobility, and safety
- Educate patients and caregivers on home programs and adaptive techniques
- Maintain accurate and timely clinical documentation
- Collaborate with a multidisciplinary care team to ensure high-quality, coordinated care
