Certifies clients for participation in the WIC Program in accordance with the policies of the State WIC Program and regulations stated in DHM 150-24. Provides high and low risk nutrition education contacts as defined by agency policies. Travels to other outlying clinics as needed and maintains the same client service standards as in the main clinic. Correctly documents nutritional assessment and care plans in medical records using SOAP or other approved formats and provides nutrition counseling, education, and breastfeeding education using approved materials and format. Assists clients in setting goals for behavioral change and evaluates client progress in meeting goals. Accurately completes reports and data entry into FL-WiSE by required deadlines, using WIP manual and DHM 150-24. Participates in preparation and presentation of outreach, bulletin boards, wellness programs, in-service training, and other activities as assigned by supervisor. Attends staff meetings and professional development activities 90% of the time. Provides service to clients in an efficient manner to assure smooth clinic flow and even caseload distribution among nutrition staff. Adheres to department policies for sanitation, safety, phone and computer use. . Performs other related duties and responsibilities as assigned. May be required to use personal vehicle for transportation to assignments.
