About The Position

Primarily responsible for the management and oversight of professional nursing assessment to determine health needs for individuals enrolled with LIFE Pittsburgh, care plan development and review, and monitoring of nursing interventions. This includes providing direction and evaluation of nursing performance, and adherence to established standards, regulations, policies and procedures to ensure provision of safe, quality care. The Nurse Manager determines staff schedule requirements on a 24 hour coverage basis and works collaboratively with the Center Administrator to facilitate coordination of services. The Nurse Manager also provides center leadership for all members of the team, in the absence of the Center Administrator. About LIFE Pittsburgh LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric physicians and nurse practitioners, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in-home program according to an individual Participant's needs.

Requirements

  • Must possess an RN license
  • five years of experience working with geriatric patients in a health facility or community- based setting
  • one to three years in a progressive management position required
