About The Position

In compliance with regulation, company policy, and excellent nursing practice, the LPN is responsible for supporting wellness functions that may include, but is not limited to: documentation on electronic health records, communication with residents, families, vendors, physicians, pharmacy and staff; performing and monitoring health metrics for wellness and supporting the care team in addressing resident care issues and concerns. Facilitate Enriching Community Environment: • Record the resident’s vital signs e.g. (blood pressure, temperature, weight, etc.) • Support ADLs for residents and the resident care team • Manage medication orders and documentation in compliance with regulations and follow up as needed with treating physicians and pharmacy • Develop collaborative relationships with third-party service providers like social workers and therapy, to ensure that necessary medical services are received by the resident • Educate residents and families in proper healthcare procedures • Documentation of staff training requirements • Delivery of staff training to support resident care • Provide direct assistance to residents with ADLs. ADLs include medication management, dietary management, personal hygiene, and other skills that enable independence • Responsible for ordering and checking medications from the pharmacy to the medication supply area • May conduct quarterly medication audits for compliance • Models strong belief in mission and core values • Active participation in learning each resident’s interests/preferences to recognize a change in condition • Responsible for ordering, securing, and maintaining stock supplies for the care department as well as maintaining cleanliness and order • Executes good judgment in the best interest of customers. Explains and educates. • Effectively resolves resident concerns through consistent problem resolution in alignment with policy • Participates in work safe environment through workplace safety policies including PPE, fire extinguishers, and SDS • Contributes to an environment of belonging for residents, families, and employees • Models flexibility and professional conduct in adjusting priorities to meet business demands • Ensures confidentiality in alignment with HIPAA, state/federal regulations as well as company policy • Other duties as assigned Physical Activity & ADA Requirements • Standing for 4 to 6 hours at one time within 8 hour day • Sitting 1 to 2 hours at one time within 8 hour day • May support/assist up to 50 lbs frequently and 150 lbs occasionally • May lift/carry 40 lbs frequently and 70 lbs occasionally; the height of 3 to 4 ft for a distance of 30 yds • Repetitive use of hands for simple grasping, pushing, and pulling, fine manipulation • Bend: frequently • Squat: frequently • Kneel: frequently • Climb: frequently • Reach: frequently, 3 feet

Requirements

  • Enthusiasm for life and adding value to the lives of others
  • High School diploma/ GED required
  • Current state LPN license
  • Current CPR certification and First Aid certification
  • At least 2 years of experience working with seniors
  • Organized with demonstrated follow through with detail
  • Strive for excellence. Previous experience focusing on customer service
  • Ability to influence others in cooperation to attain goals
  • Lead by example in practicing good judgment in relation to confidentiality
  • Proven ability to juggle multiple priorities with success and find joy
  • Great communication skills to interact with various customers in conversations and writing
  • Ability to establish confidence and trust
  • Desire to act like an owner in day-to-day decision-making and problem-solving as well as accountability
  • Demonstrated proficiency with computer programs such as Microsoft Office and the ability to learn specific software
  • Ability to adapt the schedule to work weekends, evenings, and flexible hours to ensure a quality customer experience and to meet deadlines
