Requirements
- Active, valid, NYS RN license
- Minimum 2 years of nursing experience
- Strong organizational and coordination skills
- Passion for community-based care and person-centered support
Nice To Haves
- OPWDD experience is a major plus
Responsibilities
- Conduct health assessments and ongoing monitoring for the people we support
- Coordinate and schedule medical appointments and follow-ups
- Partner with our Medical Services Team to ensure seamless, high-quality care
- Collaborate with interdisciplinary teams to support person-centered outcomes
- Advocate for and promote the overall health, wellness, and independence of the people we support
