About The Position

A primary role of the Nurse Supervisor is to work within the local community and SCO Family of Services to assure that the NFP Program is implemented in accordance with established guidelines and contract requirements. In addition, the Nurse Supervisor uses principles of supervision, reflective practice, and staff development to provide primary support and appropriate oversight to the Nurse Home Visitors, and to build and model a strengths-based, client-centered culture.

Requirements

  • BS in Nursing required.
  • Current nursing license (in good standing).
  • Previous experience in public/community health, including home visitation.
  • Previous experience providing maternal/child health services, particularly in community settings.
  • Able to establish and maintain relationships based on trust, support and growth with team colleagues and community members.
  • Works collaboratively with community providers.
  • Effective at promoting cohesion and a sense of community within the nursing team/admin. staff.
  • Responsive to clients, staff, community partners and funders.
  • Be team players.
  • Have a strong sensitivity to cultural differences present among staff and clients within our organization.
  • Possess a strong belief in people’s ability to grow and change; forge a mutually respectful partnership with people served and their families.
  • Ability to set limits and maintain helping role of practitioner and to intervene appropriately.

Nice To Haves

  • Master’s degree in nursing or closely related field preferred.
  • Prior program management and one-to-one supervision experience preferred.

Responsibilities

  • Oversee the recruitment, hiring and training of all Nurse Home Visitors and administrative staff ensuring that they can implement the NFP philosophy and model.
  • Ensure that all staff attend all required NFP education sessions as outlined in the orientation materials and contract with the Nurse-Family Partnership National Service Office.
  • Ensure that supervision of NFP home visitors utilizes principles and concepts of motivational interviewing, coaching and reflective practice (modeling therapeutic relationships, change theory, self efficacy, client-centered, strengths based and solution focused approaches).
  • Implement regular reflective practice supervision within the program and conduct field visits, both with frequency recommended by NFP.
  • Build confidence and skill of home visitors through weekly one-on-one supervision, facilitated case conferences, constructive feedback, and supplemental education.
  • Assess and develop strategies to address knowledge and skill gaps of home visitors and support staff.
  • Provide adequate orientation, professional development opportunities and study time to remediate gaps in necessary knowledge and skill.
  • Assign client caseloads and monitor home visitor’s ability to build and manage caseload.
  • Provide phone consultation to nurse home visitors while in the field, utilizing the program director and other resources as needed.
  • Provide useful developmental feedback and create a plan with staff to address and resolve performance issues.
  • Utilize agency policies and procedures to guide implementation.
  • Participate in all quality improvement processes.
  • Oversee and ensure timely and accurate documentation and data entry into electronic medical records.
  • Review and interpret NSO-generated data reports; share appropriately with team and partners.
  • Monitor program data/outcomes and implement quality improvement initiative to improve outcomes
  • Provide routine surveillance of potential and known client care incidents.
  • Participate in program development opportunities and activities as requested.
  • Maintain organizational structure and systems adequately so that home visitors may stay focused on clients, including management of inventory of supplies, educational materials, and incentives.
  • Maintain consistent communication with Program Director to minimize disruption to any areas of the program and maximize program’s ability to be proactive and responsive.
  • Promote team cohesion and maintain a supportive team environment
  • Collaborate with community partners to strengthen program impact
  • Represent program in meetings and other outreach activities
