Job title: Program Manager - Nurse Family Partnership The Program Manager for the Nurse-Family Partnership (NFP) oversees daily operations to ensure effective delivery of the evidence-based home visitation program. This role is responsible for staff supervision, program implementation, quality assurance, data monitoring, and compliance with funding and contractual requirements. The Program Manager collaborates with the National Service Office (NSO), community partners, and internal teams to support client outcomes, maintain model fidelity, and guide program improvement. The position also develops policies and procedures, manages budgets, and supports strategic planning to ensure program sustainability and growth.

Requirements

  • Bachelor's degree in nursing, master’s preferred
  • One to five years of pediatric nursing practice with a minimum of 3 years of supervision experience is required.
  • Auto Insurance Certificate Required
  • CPR and First Aid Certification Required
  • DES Criminal Affadavit Required
  • AZ DHS Criminal History Affidavit Required
  • Driver License Required
  • Fingerprint Card Level 1 Required

  • Previous experience in maternal/child health and/or community health nursing and home visiting is desired

Responsibilities

  • Manage daily operations of the NFP program, ensuring adherence to the national NFP model and fidelity to core elements.
  • Conduct regular data reviews, prepares for, and participates in site visits, and maintains communication with the NSO.
  • Serve as the liaison to contracting agencies, funders, and NSO to ensure compliance with deliverables, scopes of work, and performance benchmarks.
  • Develop and implement program policies and procedures to support consistent, high-quality service delivery and operational efficiency.
  • Recruit, train, supervise, and evaluate Nurse Home Visitors.
  • Lead quality improvement initiatives in coordination with NSO and funders.
  • Monitor service delivery, staff productivity, and client outcomes using ETO and other data systems; analyze trends to guide program improvement.
  • Integrate data into staff practices and program evaluation.
  • Oversee NFP grant administration, including budgeting, procurement, expense tracking, and resource allocation.
  • Coordinate with the finance department on reporting, audits, and fiscal planning.
  • Establish and maintain referral relationships with clinics, hospitals, and community agencies.
  • Organize and facilitate quarterly Community Advisory Board (CAB) meetings per NSO standards.
  • Implement outreach and enrollment strategies to meet caseload targets and enhance program visibility.
  • Ensure compliance with HIPAA, state and federal regulations, and contractual requirements.
  • Submit timely and accurate reports to funders, NSO, and internal stakeholders.
  • Coordinate responses to audits, performance reviews, and contract changes.
  • Participate in long-term planning to sustain and grow NFP services.
