About The Position

Nurse Specialist are responsible to prioritize/manage their time/resources as applicable to the needs of the outpatient and/or inpatient settings. They Coordinate communication between Physicians and Medical staff members, other outpatient and inpatient departments/staff, patient and family. They gather information regarding their implanted device, coordinate device interrogations for both outpatient and inpatients, interrogate and/or change settings on implanted devices, manage all remote interrogation and alert reports from the company websites and in PaceMate system. Perform all aspects of documentation in Cerner and Revenue Cycle. Hours: Full-time/40 hours per week Required: ADN; RN; BLS or CPR within 90 days of hire. Pay: Based on experience, starting at $33.73

Responsibilities

  • Analyzes cardiac rhythms for patient monitored with Implantable Loop Recorders, Pacemakers, ICDs and other devices monitor in Device Clinic. Triages and documents findings and notifies appropriate provider.

  • Assessment - Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data.
  • Assists in rehab classes as needed by performing vitals, watching telemetry and responding to emergencies in gym.
  • Charts, bills and schedules tele traces, remote pacemaker, ICD and implantable loop recorder transmissions.
  • Contacts patients to respond to questions, troubleshoot monitors and query about symptoms when necessary. Assists in scheduling patients for follow up or replacement as indicated.
  • Coordination of Care - Plans and evaluates care in collaboration with appropriate disciplines.
  • Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues.
  • Directs and guides other health care workers and assumes accountability for performance of nursing activities delegated to other workers.
  • Educates patients and families regarding device function and remote and/or clinic follow up scheduling.
  • Establishes and maintains an ongoing evaluation of pacemaker function for each patient based on date of implant and type/mode of pacemaker through tele trace transmissions.
  • Establishes and maintains communications with patients, families, significant others, and health care teams and promotes participation regarding the plan of care.
  • Evaluation - Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to others as appropriate.
  • Implementation - Implements care or work plan in alignment with the plan and approved safety, infection control, and department/organization standards.
  • Knowledge - In collaboration with leaders, actively pursues required knowledge and skills through orientation activities specific to the position, reading current literature and seeking new learning opportunities.
  • Maintains advanced and in depth knowledge/understanding of the device technology, the purpose of device implant, expected alteration in clinical condition, and potential device related causes for patients symptoms. Effectively and consistently troubleshoots device problems in the wide array of implantable device types implanted.
  • Maintains understanding of the interaction between programming changes and the potential clinical outcome, and collaborates with MD to ensure best programming.
  • Manages care for pacemaker and heart failure patients in a timely and cost effective manner. Demonstrates clinical competence while providing care.
  • Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situations.
  • Oversees all aspects of care provided through Pacemaker Services, including scheduling & registration of patients, provision of care, billing and documentation of tele trace and clinic visits.
  • Participates in activities to enhance individual and professional leadership development.
  • Participates in quality management and PI programs to improve processes and outcomes. Participates in evaluation of patient care delivery system and self-evaluation.
  • Performs cardiac stress testing, including documentation of history and medications, assessing vital signs during testing, calculating medication dosage and preparing and administering medication, monitoring patient's medical status, and performing ACLS during emergency situations.
  • Performs pacemaker interrogations, evaluations, and changes pacemaker parameters per MD order.
  • Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes.
