Our wonderful nursing staff meets our patients and families where they are, private homes, nursing facilities, assisted living facilities, or our free standing 24 bed inpatient center. Your Life as a Nurse Practitioner Your role is very important to our patients and organization. We meet our patients where they are and this position will serve our patients in all counties. We will rely on you to ensure that all patients you visit have addressed any nursing needs or determine the need for other team members to assist in providing resources. To achieve this you will assist the patient or family in determining the best plan of care that meets their needs. We are a team and it is your role to direct other team members and ensure that they are exceeding our patient's expectations. We want to share your extraordinary care and need to make sure that we are able to process billing. You are responsible for documenting your care and ensuring your team members are as well. This documentation should paint the picture of your patient, so if another team member is given the honor of serving that patient they are aware of their expectation and know what is currently working. Your focus will always be on what is your patient's greatest need and providing "Care, Hope, and Support for all when Illness threatens life." This position provides is designed to have flexibility in continuing care while other team members are out or supplement high levels of admissions. As a Nurse Practitioner, your goal should be that your patients experience is their loved ones memory.

Requirements

  • Graduate of an accredited school of medicine or osteopathy.
  • Practicing Nurse Practitioner, currently licensed under the provisions of Texas Medical Practices Act.
  • Understand the overall objectives and philosophy of Hospice care.
  • Understand the physical, psychosocial and spiritual concerns/problems of terminally ill patients and families.
  • Ability to work effectively with community physicians.
  • Ability to work effectively within an interdisciplinary team.
  • Ability to provide medical education to hospice team members, community physicians, medical schools and teaching hospitals.
  • Demonstrated competency in defined areas.

Nice To Haves

  • One year of previous Hospital experience preferred.

Responsibilities
