The Nurse Practitioner PRN plays a critical role in delivering high-quality, patient-centered healthcare on an as-needed basis across various clinical settings. This position involves assessing, diagnosing, and managing acute and chronic health conditions while collaborating closely with physicians and other healthcare professionals to ensure comprehensive care. The Nurse Practitioner will provide health education, counseling, and preventive care to promote patient wellness and improve health outcomes. Flexibility and adaptability are essential, as the role requires responding to fluctuating patient volumes and diverse clinical demands. Ultimately, the Nurse Practitioner PRN contributes to maintaining continuity of care and enhancing patient satisfaction through expert clinical practice and compassionate communication. As a Hospice Nurse Practitioner, you will: Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease. Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed. Perform Face-to-Face visits at the patient's place of residence. Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians. Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned. Keeps up with and maintains education/knowledge in pharmacology and palliative medicine. Knowledgeable of hospice criteria.
