Requirements

Education: Master’s or Doctoral degree in Nursing (FNP or AGNP focus).
Licensure: Active New York State NP License; Board Certification required.
Experience: New graduates are welcome; prior experience in geriatrics or nursing homes is a plus.
Technical Skills: Proficiency in EHR systems, chronic disease management, and infection control protocols.
Soft Skills: Strong communication, empathy, cultural competency, and attention to detail.

Responsibilities

Conduct comprehensive patient assessments, including medical history reviews and physical exams.
Develop and update individualized care plans for residents with acute/chronic conditions.
Monitor and manage infections by adhering to facility protocols.
Educate patients and families on disease management, prevention, and wellness.
Collaborate with physicians, nurses, and therapists to ensure coordinated care.
Maintain accurate documentation of treatments, progress, and care plans.
