About The Position

Mend Health is seeking a skilled and compassionate Arrive Home Care Nurse Practitioner to join our growing team. As a Nurse Practitioner specializing in Care Management including transitional and chronic care, you will play a crucial role in delivering high-quality healthcare services to our patients in various home settings. You will be responsible for assessing, treating, and managing acute and chronic illnesses, collaborating closely with patients, families, and our multidisciplinary team to promote healing and optimize patient well-being.

Requirements

  • Minimum of 3-5 years of clinical experience as a Certified Nurse Practitioner (CNP) in direct patient care with experience in treating medically complex patients with multimodal care including pharmacologic and non-pharmacologic therapy
  • Proficiency in performing assessments understanding disease process of chronic illnesses
  • Strong interpersonal and communication skills, with the ability to effectively educate and empower patients and caregivers
  • Ability to work independently and collaboratively within a multidisciplinary team environment
  • Compassionate, patient-centered approach to care, with a commitment to delivering high-quality healthcare services
  • Master’s degree in nursing from an accredited Nurse Practitioner program
  • Active, unrestricted Certified Nurse Practitioner (CNP) license in the state of practice, with national certification as a Nurse Practitioner in Adult-Gerontology, Family, or Acute Care

Responsibilities

  • Conduct focused or comprehensive assessments for post-acute patients recognizing chronic illnesses, including heart disease, diabetes, COPD, and other long-term illnesses
  • Identify opportunities to collaborate and develop clinical integration opportunities with patients and other health delivery systems to achieve clinical and operational outcomes
  • Provide medical and clinical guidance, support and education to the clinical nurse and medical assistants
  • Educate patients, caregivers, and families on care management including motivational interviewing techniques, preventive measures, and signs of exacerbation or complications
  • Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care and optimize treatment outcomes
  • Ensure compliance with medical and clinical goals through monitoring care management performance
  • Maintain accurate and up-to-date medical records, including wound assessment documentation, treatment plans, and progress notes
  • Participate in continuous education and training to stay current with advancements in Chronic Care Management and technologies.
  • Work closely with the Medical Director and other key staff to develop and implement care model enhancements
  • Adhere to Mend Health’s policies, procedures, and regulatory requirements, ensuring compliance with standards of practice and patient safety protocols.
  • Contribute to the leadership team and play a significant role in the Company’s growth strategy
