The Nurse Practitioner will deliver primary care services within designated nursing home facilities and through scheduled house-call visits to homebound patients in Manhattan. Requirements
- Nurse Practitioner License – NY (Active)
- Ability to work independently and manage a mobile schedule
Nice To Haves
- Experience in geriatrics, nursing home care, internal medicine, or home-based primary care preferred
- Strong communication and clinical assessment skills
Responsibilities
- Conduct routine and acute patient evaluations in nursing home settings and during home visits
- Develop treatment plans and manage chronic conditions
- Collaborate with facility staff, physicians, and care coordinators
- Maintain accurate documentation and follow regulatory guidelines
- Provide compassionate, patient-centered care to seniors and homebound individuals
