About The Position

EvergreenHealth is currently recruiting a full time (1.0 FTE) Hospice Advanced Nurse Practitioner to join our exceptional and growing Hospice Care Team with our hospice home care team in Kirkland, WA. Requirements

Hospice or Palliative Care experience required.

Responsibilities

Provide medical oversight for hospice patients in collaboration with hospice physicians.
Perform face-to-face visits for hospice patients.
Participate in interdisciplinary team and hospice medical provider meetings.
Participate in quality improvement projects and provide staff education and support regarding end-of-life care.
Perform other duties as indicated.
