About The Position

Lower Cape Fear LifeCare is an award-winning nonprofit hospice and palliative care provider with over 40 years of service in southeastern North Carolina and northeastern South Carolina. This full-time Nurse Practitioner role is budgeted for 40 hours per week, with flexibility in exact working hours. The position involves managing a caseload with a goal of at least 20 visits per week, averaging 4-5 visits daily, depending on patient needs and location. Visits typically last 1 to 1.5 hours, with flexibility to reschedule based on patient acuity and regulatory guidelines. The role is not an emergency service, directing urgent situations to Primary Care Providers.

Requirements

  • Graduate of an accredited school with a Master’s Degree in Nursing.
  • Current certification from the American Academy of Nurse Practitioners (ANNP) or American Nurses Credentialing Center (ANCC).
  • Current license or ability to obtain licensure from the SC Board of Nursing (NP).
  • Minimum of five years general medical experience in inpatient, outpatient or nursing home settings.
  • Minimum two year experience as a nurse practitioner.
  • Demonstrated competence in leadership and teaching.

Nice To Haves

  • Certification in hospice/palliative care.
  • Experience seeing patients in a home setting.

Responsibilities

  • See patients in various venues of care including personal residences, assisted living facilities, skilled nursing facilities, clinic, and telehealth.
  • Provide quality care to patients from diverse socioeconomic and cultural backgrounds.
  • Educate caregivers on patient care, including disease education, advance care planning, medication administration, and end-of-life expectations.
  • Spend extended periods driving between patient visits, documenting, and coordinating care.
  • Coordinate care with community medical providers and other disciplines to provide updates and obtain new orders.
  • Self-schedule visits based on patient needs and acuity.
  • Participate as a member of an Interdisciplinary Team (IDT) comprising a physician, nurse practitioner, registered nurse, and medical social worker.
  • Attend IDT education rounds twice a month.
  • Attend monthly palliative care staff meetings.
