Outpatient Nurse Practitioner - .4 FTE Park Nicollet Methodist Outpatient Hospice Park Nicollet is seeking a part-time Nurse Practitioner (.4 FTE - Thursdays and Fridays) to join our outpatient hospice team. Being a part of our team means you will have an impact on the care that our patients receive every day. We are seeking an outstanding nurse practitioner to join a thriving outpatient hospice team. Join a dynamic and dedicated interdisciplinary team of physicians, advanced practice clinicians, nurses, social workers, music and massage therapists and chaplains in providing great patient centered care to patients with serious illness. This role will function primarily in the outpatient setting, accessing needs of hospice care patients and families, assisting in the management of complicated symptoms and goals of care discussions as well as face to face home visits. Palliative care education and experience preferred. Excellent teamwork, symptom management and communication skills are a must.
