Harlem United is the parent company for Upper Room AIDS Ministry (URAM). URAM provides the healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service. The Nurse Practitioner assists with leading the care team and provides guidance to all team members. The Nurse Practitioner is expected to help lead the care team with a commitment to ensuring each team works to the top of his or her license consistently adheres with all protocols. It is expected that the Nurse Practitioner will delegate visits that do not require a physician or Nurse Practitioner to the RN allowing the Nurse Practitioner to dedicate more time to those patients that need to be seen with the scope of practice for the Nurse Practitioner. Nurse Practitioner’s are also expected to ensure that each patient has an individualized care plan with oversight by the PCP and that can be used by each care team member to help facilitate and guide care. Nurse Practitioner are also responsible to provide excellent customer service at all times. Under the supervision of the Medical Director and in accordance with established policies and procedures, the Nurse Practitioner delivers high quality comprehensive primary care, with expertise in HIV, immigrant and homeless care.
