About The Position

This role is an integral part of the care delivery system, focused on providing a healing environment and prioritizing customers. The incumbent delivers primary healthcare services for non-acute or chronic health conditions to specific populations, adhering to applicable scope and standards of practice, and the organization's policies, values, and mission within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, and includes weekend and on-call coverage on a rotational basis as required. In addition to base compensation, this position offers an annual quality-based bonus tied to contributions to population health initiatives and value-based metrics, rewarding dedication to a team-based approach, improving patient outcomes, and driving efficient, high-quality care.

Requirements

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program
  • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB)
  • Practicing-state licensure as a Nurse Practitioner
  • Prescriptive Authority from the practicing state
  • Within 60 days of hire Drug Enforcement Agency Licensure (for Nurse Practitioner)
  • Within 60 days of hire CPR – American Heart Association (for Nurse Practitioner)
  • Act 34 Criminal Background Clearance Certificate (for Nurse Practitioner)
  • Act 33 Child Abuse Clearance Certificate (for Nurse Practitioner)
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate (for Nurse Practitioner)
  • Master’s degree in Physician Assistant Studies
  • Current PA Physician Assistant License
  • PA-C or board eligible.
  • Ongoing successful recertification (NCCPA) as a PA-C
  • Within 90 days of hire Drug Enforcement Agency Licensure (for Physician Assistant)
  • Within 60 days of hire CPR - American Heart Association (for Physician Assistant)
  • Act 33 Child Abuse Clearance Certificate (for Physician Assistant)
  • Act 34 Criminal Background Clearance Certificate (for Physician Assistant)
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate (for Physician Assistant)
  • Four to ten years related experience and/or training; or equivalent combination of education and experience.

Responsibilities

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
  • Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care
  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
  • Triages patient telephone calls, provides monitoring and continuity of care
  • Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.
  • May perform other duties such as manage outpatient concerns and any other duty specific to specialty.
  • Other duties as assigned or requested.
