About The Position

The Nurse Manager directs the health care services staff to ensure the delivery of quality health services to youth at a site or in a particular service. This role may also be responsible for providing health care oversight and medical case management for an identified caseload of youth, as well the clinical functions of the Registered Nurse (RN). This is a full-time position, primarily on Monday - Friday days, with flexibility for being on the on-call rotation required. This role oversees the residential nursing teams at both the Monroe Campus and Golisano Campus at Crestwood. It is supported by an Assistant Nurse Manager at each campus and the Residential Nursing Director.

Requirements

  • Successful completion of an accredited Associate or Bachelor’s Degree Nursing Program required.
  • Minimum 2 years of experience as a Registered Nurse required.
  • Minimum 1 year of leadership experience required.
  • Active, unrestricted RN license in NYS required.
  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Basic Life Support (BLS) training within 60 days of hire and recertification annually.

Responsibilities

  • Ensure the health care provided to the youth in care is compliant with all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to NYS Office of Mental Health (OMH), NYS Office of Child and Family Services (OCFS), NYS Office of People with Developmental Disabilities (OPWDD), NYS Education Department NYSED), NYS Department of Health (DOH) and Council on Accreditation (COA).
  • Develop and deliver coordinated health services in accordance standards of professional practice as part of an interdisciplinary team.
  • Understand and assist with the development of all health care service delivery systems within the assigned location.
  • Maintain Full-Time Equivalents (FTEs) within budgeted parameters and manage expenses within the assigned health care services program.
  • Support process improvement initiatives in collaboration with agency leadership.
  • Conduct ongoing quality assurance reviews for nursing systems and nursing staff.
  • Monitor and respond to patient movement, including referral, admissions, health care and discharge.
  • Advocate on behalf of youth in care to establish and maintain appropriate health care.
  • Perform all clinical functions of a Registered Nurse (RN) for youth admitted to assigned programs, when needed.
  • Create and maintain day to day staff scheduling to meet regulation and patient care requirements.
  • Attend meetings related to the admission process to obtain medical/psychiatric history, establish health care needs, and complete prior authorization for medications or medical necessity based on recommendation from medical and psychiatric team, when needed.
  • Review health care recommendations from youth’s physician with the treatment team and family to encourage family participation and implement follow-up care recommendations.
  • Consult with pediatrician or other members of the patient’s health care team to follow up on physical and psychiatric medical concerns.
  • Provide information for treatment, residential, program coordinators, case management and other agency staff for health service follow-up.
  • Provide education to staff and patients on relevant consumer health topics, to help ensure proper care and follow-up.
  • Document in each patient’s electronic clinical record all diagnostic assessments, individual plans of treatment, and treatment notes regarding recommended services and appropriate treatment.
  • Provide discharge summary establishing health care follow-up, when needed.
  • Function regularly as a preceptor for new staff and contract nursing staff.
  • Consult and coordinate with treatment team providers such as the treating physician, nurse practitioner, and clinician, as well as others to assess, plan, implement, and/or evaluate patient care plans while assuring continuity and quality of care.
  • Provide nursing services, such as case finding, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being to youth admitted to assigned programs.
  • Function as a member of the local campus based or service-based leadership team.
  • Make sound critical decisions and have authority to implement decisions in conjunction with the leadership structure of the given area of responsibility.
  • Maintain and adhere to ethics of confidentiality. Discuss confidential matters in an appropriate manner in compliance with the Health Insurance Portability Accountability Act (HIPAA).
  • Assume on call responsibilities on a rotating basis as per service guidelines, as assigned.
  • Maintain effective and professional relationships with assigned contacts at Temporary Staffing Agencies as appropriate to ensure that contract staff is retained as needed and all required onboarding and payroll processes are followed.
